Nex Gen Youth are always required to have a CCC Youth Ministry Activities Medical & Liability Release Form signed by their parent(s) or guardian(s) before attending any event without the accompaniment of their parent(s). (Waiver below for download)
Please email signed form to jcampertrinity@gmail.com or you can send your child with the form, but they cannot attend without the release, so we would prefer if you email it ASAP, thank you.
